Bile duct stricture is a complex problem. Narrowing of the bile duct results in obstruction to the flow of the bile flow from the liver to the intestine. This can occur because of benign (non-cancer) or malignant (cancer) conditions. Common benign conditions are complications of gallstones, injury to the bile duct during gallbladder surgery, peptic ulcer, primary sclerosing cholangitis, liver parasites, etc. Cancer of the bile duct at different levels of the bile duct from the liver to the intestine.

Benign strictures can occur following previous biliary surgery where the bile duct has been connected to the intestine and undergoes narrowing. We had managed recently a patient who had a previous Whipple operation and presented more than a decade later with jaundice. After abdominal scans (CT and MRI/MRCP) the patient was diagnosed to have cancer of the proximal bile duct and advised to undergo a major liver resection surgery for cure, at another hospital.

A careful review of scans raised the suspicion of benign stricture of hepaticojejunostomy with stones above which was misinterpreted as cancer previously. The diagnosis was confirmed on further evaluation by prcutaneouscholagiogram (specialized test to study the bile duct system). Since this narrowing could not be dilated by the intervention radiologist, a revision of the bile duct to the jejunum connection was done by laparoscopic surgery. We routinely get referrals of patients with bile duct strictures that occur following laparoscopic cholecystectomy. These strictures need special expertise in recognizing the type of bile duct strictures and careful planning and execution of surgical repair.
